I, the undersigned, voluntarily choose to participate in the CrownWork™ Block Party: Wellness Lounge event hosted by Curl Rhythm® on March 21, 2026. I understand that activities at the event may include, but are not limited to: massage, red light therapy, line dancing, and consumption of food and beverages.
Assumption of Risk. I acknowledge that participation in these activities carries inherent risks, including but not limited to physical injury, allergic reactions, skin sensitivity, muscle soreness, or other adverse effects. I voluntarily assume all risks associated with my participation.
Release of Liability. I hereby release, waive, and discharge Curl Rhythm®, its owners, officers, employees, agents, contractors, wellness providers, venue staff, and all affiliated parties from any and all liability, claims, demands, or causes of action arising out of or related to my participation in the event, including any injury, illness, damage, or loss of personal property, whether caused by negligence or otherwise.
Medical Acknowledgment. I confirm that I am in good physical health and have no medical conditions that would prevent my safe participation. I understand that the wellness services provided (massage, red light therapy) are not medical treatments and are not a substitute for professional medical care. I have disclosed any relevant health conditions above.
Indemnification. I agree to indemnify and hold harmless Curl Rhythm® and all affiliated parties from any claims, damages, or expenses arising from my participation in the event.
